Core Group Sign-Ups

If you are interested in being a part of the core group that leads the church plant, please fill out the document below. While anyone is free to join this church plant, only Center Point members will be considered for the core group, who will be responsible for leadership decisions at the church plant. Not everyone who fills out this document will be chosen as a core group members.
